Privacy policy

The contact form collects five things and nothing else: your name, your email address, your phone number, how you prefer to be reached, and the message you choose to write. Email or phone is enough; you do not have to give both.

We do not ask for anything else. If you describe your health, finances or family in the message, that is sensitive information: write only what you are comfortable sharing online. The rest can wait for the visit.

The site also measures its own traffic: the page viewed and the time, tied to a temporary session identifier that disappears when you close the tab. No advertising cookies, no permanent identifier, no profiling.

To reply to you by the method you chose, to book a first visit, and to keep a record of your request in our client file so we do not ask you the same questions twice.

Your details are never used for marketing and are never sold or rented.

PLACEHOLDER: The people at Maple Physio (Test) who answer requests. Name the roles: the owner, a receptionist, an assistant. Nobody else.

Providers who act on our behalf, under contract, and who do not use your information for their own purposes: VisualFlow Analytics, which hosts this site and the client file; the service that delivers the notification email; and the database host.

PLACEHOLDER: Our providers may store data on servers outside your province or country. We have assessed that transfer as the law requires.

PLACEHOLDER: 12 months after our last exchange if no appointment follows; otherwise, for as long as the client file is kept.

You can ask us to delete your request at any time; we remove it within 30 days unless a legal obligation requires us to keep it.

You can see the information we hold about you, have it corrected, withdraw your consent and ask for it to be deleted. Write to the person named below; we answer within 30 days.

If you are not satisfied with our answer, you can file a complaint with the privacy regulator for your province or country. In Quebec that is the Commission d'accès à l'information.

Data travels encrypted (HTTPS) and the client file sits behind a named account. If a confidentiality incident presents a risk of serious harm, we notify you and the regulator, as the law requires.
